    OVERVIEW: Offers assistance for older adults (60+) and individuals with disabilities. It is a statewide resource for information and assistance about long-term services and supports to allow individuals to remain at home and plan for future needs.  

    1) AGING AND DISABILITY RESOURCE CENTER: Provides individuals who are seeking long term support information and counseling regarding available long term support options, and determining eligibility for public long term support programs and benefits. Benefits screening includes level of care determinations for Medicaid nursing facility and home and community-based service waiver programs.

    2) INFORMATION AND ASSISTANCE: Offers in person assistance to individuals, families and caregivers with questions and/or concerns. Available Mon-Fri, 8:30AM-4:30PM. Also, connects individuals with information and referrals to other programs and benefits (such as health promotion and disease prevention, transportation services, housing and income support programs, utility cut-off assistance, eviction prevention, prescription medication expense assistance) that may help people remain in the community.  

    3) COMMUNITY PERSONAL ASSISTANCE SERVICES (CPAS): Provides personal care services in the community for persons with chronic illness or disability.

    4) EMERGENCY FOOD: Provides case-by-case assessments for senior citizens who may be in need of emergency food. Also assists homebound seniors 60+ through the Meals on Wheels program.

    5) SENIOR FARMERS’ MARKET NUTRITION PROGRAM: Provides vouchers to low-moderate income older adults, 60 and older, for use at local farmers’ markets in the community. Sign-up begins July to eligible older adults at local Senior Centers on a first come, first served basis. Call for individual senior center voucher distribution information. 

    6) CAREGIVER SERVICES: Provides education, respite care, training and support services for caregivers.

    7) SENIOR CARE: Offers persons 65 and older services to assist them remain in their homes for as long as possible. Services include adult daycare, in-home aide services, medical supplies, emergency response systems, and prescription assistance.
