    OVERVIEW: Provides a wide range of services to Talbot County residents that consists of family support, addictions, mental health, wellness and community awareness through local outreach.

    SERVICES:
    1) ALCOHOL AND OTHER DRUG ABUSE PREVENTION: Assists community groups, agencies and individuals in providing programs and activities to prevent substance abuse and to build a healthier community.

    2) NALOXONE TRAINING: Offers an educational program that includes an understanding of opioids, recognizing signs and symptoms of an overdose, proper rescue breathing techniques, proper administration of naloxone, and care for the individual until the paramedics arrive. Individuals must be 18 and older related to and/or working with individuals dealing with opiate substance use. Provided to family members, caregivers, guardians, and health/human service workers. Call for the Overdose Response Training days and times.
      
    3) MEDICAL ASSISTANCE TRANSPORTATION: For Medical Assistance Clients only. Transports eligible ambulatory and wheelchair clients to their medical appointments.     4) COMMUNICABLE DISEASES: Services are provided for adult and child immunizations, for uninsured and under insured, STD's and tuberculosis screenings and community education.

    5) DEVELOPMENTAL DISABILITIES PROGRAM: Provides integral support services and case management to individuals with developmental disabilities and their families.

    6) ENVIRONMENTAL HEALTH PROGRAM: Provides food protection, sewer program, water program and rabies program. Location: 215 Bay Street, Suite 4. Easton, MD 21601. 410-770-6880; fax: 410-770-6888.

    7) FAMILY PLANNING PROGRAM: Offers confidential education, physical exams and contraceptives to females and males who wish to prevent pregnancy.

    8) ADULT EVALUATION AND REVIEW SERVICES: Evaluates health-impaired persons at home and either develops a plan of care to maintain them in the community or assists with appropriate placement.

    9) HIV/AIDS PROGRAM: Offers free and confidential anonymous HIV testing, case management, referrals for services for HIV-infected clients and HIV education for community groups.

    10) IMMUNIZATIONS: Provides routine immunizations by appointment to adults and children who are uninsured and underinsured. Includes vaccines for influenza, hepatitis
    A and B, pneumococcal, meningococcal, and rabies.

    11) MEDICAL ASSISTANCE (PERSONAL CARE) PROGRAM: Provides in-home care for Medical Assistance recipients who require assistance with activities of daily living in order to remain in the community.

    12) SCHOOL HEALTH PROGRAM: Helps children get the most out of school by preventing, detecting and treating conditions that might interfere with learning. Several sites have primary care services through school-based clinics.

    13) SENIOR CARE: Provides ongoing case management and in-home services for health-impaired persons ages 65 and older, who are at risk of nursing home or other institutional placement.

    14) TOBACCO PREVENTION PROGRAM: Offers nicotine replacement therapy, as well as free smoking cessation classes and educational presentations at various locations in the county and at work sites.

    15) TALBOT COUNTY EARLY HEAD START: Offers parenting skills, GED and support services to parents with children under the age of 4.

    16) CHILDREN AND YOUTH WITH SPECIAL NEEDS HEALTH CARE: Provides resource coordination, information about financial assistance, and supportive services. Must be 21 or under, have a chronic illness, physical disability or health related or behavioral problems that interfere with normal growth and development.

    17) CANCER PREVENTION, EDUCATION,  & SCREENING: Offers colorectal screenings for qualified patients. Also offers education on colorectal, breast, cervical, and skin cancers.

    18) LEAD PREVENTION AND OUTREACH: Provides early identification and intervention for children at risk for an exposed to environmental lead. Children with at least one risk factor receive a blood lead screening.

    19) HEALTHY FAMILIES: Provides coordination or care in efforts to improve birth outcomes and reduce infant mortality and morbidity for Medicaid/MCHP eligible pregnant women and infants.

    20) BIRTH CERTIFICATES: Provides certified copies of birth certificates to individuals born in the state of Maryland in 1939 or after must provide proper ID and complete form. There is a $22 fee. Certificates for individuals born before 1939 must be mailed along with copy of valid photo ID & check or money order to Division of Vital Records in Baltimore, MD. For additions or correction to a certificate, call vital records unit at 410-764-3038.

    21) DEATH CERTIFICATES: Certified copies of Death Certificates are given to funeral homes and families, they must complete a form. There is a $20 fee for the 1st copy, and a $22 fee for any additional copy after that. If a certified copy is needed later than 30 days, it can be obtained at a $12 fee per copy from the vital records website at www.dhmh.state.md.us.

    22) SEASONAL FLU 2019: Call the Flu Hotline to hear dates and locations for local flu clinics. Also, visit the website for a complete listing of current local flu clinics.

    23) RABIES CLINIC 2020: (CLOSED) Pets must be at least 12 weeks old to receive vaccinations. Cost is $6 per animal and all dogs are on leashes and cats are in carriers.
